UNM Los Alamos offers its facilities to the greater Los Alamos community and other outside organizations for meetings, trainings and special events.
Please download and complete the UNM-LA facility usage form here.
Any events which will bring minors (under 18 years old) to the UNM-LA campus require the following documents:
- UNM Minors on Campus Checklist for External Organizations
- Minor Participant Waiver and Notice of Risk (must be signed by parent or guardian)
- Minor Participant Emergency Contact and Medical Release Form (must be signed by parent or guardian)
- Photography Release (must be signed by parent or guardian)
